Small Business Health Plans with 1–50 Employees

HMAA offers the following health insurance plan options to Small Businesses. The Hawaii Insurance Commissioner has confirmed the State of Hawaii definitions are as follows and will remain as such in 2016:

  • Small Group: 1 to 50 (FTEs)
  • Large Group: 51 or more FTEs

Group size is determined as follows:

  • Under the Affordable Care Act (ACA), an FTE is one who worked at least 30 hours per week during the preceding calendar year, and part-time employees are included in the calculation. Example: 3 employees each working 20 hours per week = 2 FTEs.
  • Companies with a common owner or are otherwise related are generally combined and treated as a single group, and employees are combined for purposes of determining group size.
Medical Plans for Small Businesses (with Drug and Pediatric Benefits)
  • Option Plus One (OP1) – HMAA’s most popular medical plan. This Plan helps attract and retain employees with benefits including low out of pocket maximums, Chiropractic Care, Acupuncture, Naturopathic Care, Wellness and Preventive Care, and Employee Assistance Programs. It also includes a drug co-payment plan of $5/$20/$35 with co-insurance.
  • Option Plus Two (OP2) – HMAA’s most comparable plan to Hawaii’s largest Preferred Provider Plan (PPP), with added benefits that include Chiropractic Care, Acupuncture, Naturopathic Care, Wellness and Preventive Care, and Employee Assistance Programs. It also includes a drug co-payment plan of $5/$20/$35 with co-insurance.
  • Comprehensive Plus – A lower-cost alternative to our OP1 and OP2 medical plans. This plan offers fixed copayments for office visits and minimal copayments for preventive benefits. In addition, Comprehensive Plus includes Chiropractic Care, Acupuncture, Wellness and Preventive Care, and Employee Assistance Programs. It also includes a drug co-payment plan of $5/$20/$35 with co-insurance.
  • Comprehensive Basic – HMAA’s lowest-priced plan which, like Comprehensive Plus, offers fixed copayments for office visits as well as Wellness and Preventive Care and Employee Assistance Programs. This plan is perfect for the business owner seeking affordable health insurance premiums without sacrificing quality. It also includes a drug co-payment plan of $5/$20/$35 with co-insurance.

  • Essential Health Benefits include coverage for Ambulatory, Emergency, Hospitalization, Maternity and Newborn Care, Mental Health and Substance Abuse, Prescription Drugs, Rehabilitative and Habilitative, Laboratory, Preventive, Wellness, Chronic Disease Management, and Pediatric Dental & Vision Services.
  • Pediatric Benefits for dental and vision care is provided to children through age 18.
