The Summary of Benefits and Coverage (SBC) summarizes information on certain health benefits and costs as required by the Affordable Care Act. For definitions and examples of commonly used terms, refer to the Uniform Glossary.

HMAA provides employer-sponsored plans with these documents so they may provide them to their plan participants upon enrollment or changes to plan benefits as required.
